Photo Authorization Form

By completing and signing this form, the photographer grants Matt Lipsky Design Solutions permission to use the authorized photographs for approved business, promotional, marketing, and website-related purposes.

Artist Authorization Form

This Artist Authorization Form establishes an agreement between the artist and Matt Lipsky Design Solutions regarding the use, reproduction, and display of artwork or creative materials provided to the company. By completing and signing this form, the artist grants Matt Lipsky Design Solutions permission to use the authorized artwork or materials for approved business, promotional, marketing, website, and other designated purposes.

Non-Disclosure Agreement

This Non-Disclosure Agreement establishes a confidential relationship between the client and Matt Lipsky Design Solutions. The purpose of this agreement is to protect confidential, proprietary, and sensitive information that may be shared during the course of a project or business relationship. Both parties agree to maintain the confidentiality of protected information and to use such information only for authorized business purposes.
